depends if the car is a manual or auto. manual, left hand on the wheel, 12 o clock or so, right hand on the shifter or resting on the console or vice versa and left arm out the window. auto, right hand on the wheel, left hand on the armrest/out the window

I drive with my left or right hand at the top of the wheel. It's lazy tastic so I can turn without doing the hand-over-hand business. My other hand usually sits below the steering wheel where I can grab the wheel or start an evasive action if I have to (i.e. swerve right off the road and hit a tree and die so I don't kill a squirrel or somethin).

Otherwise, right hand at 6. Aweful way to drive but I picked up the habit and never dropped it. Switching gears - left hand at 7 to 11 (depending on turn) and right on the shift (duh)
