Not sure if Georgia has the same scale as NC, but an Indian place that I like (liked) got a 36.

Some highlights:

The sink in the kitchen had paper towels
it also did not have soap
or hot water

The utensils were stored handle down, so employees always had to grab the other end (with hands that were washed without soap in cold water)

Plenty of roaches, ants etc.

And my favorite:
Food was stored in the employee bathroom...above, on top of, and UNDER/BEHIND the toilet
